About Pacifica Health

Pacifica Health Services (PHS) is the parent corporation of Carlisle Center for Wellness and Rehab and Fleur Heights Center for Wellness and Rehab.

PHS was founded in July of 2000 by a group of four Central Iowa health care providers. They represent physicians, pharmacists, and a long-term care administrator with over 100 years of combined experience in this industry.

The working philosophy of PHS is to provide high quality Independent Living, Assisted Living, Skilled Care, and Long-Term Care while maintaining a comfortable home like environment.
